Manual note-taking during and after patient visits is one of the biggest drivers of provider burnout, and it eats into time that could go toward actual patient care. We built an ambient documentation pipeline that listens to the provider-patient conversation and turns it into a structured draft clinical note, so the provider's job shifts from typing to reviewing and signing off.
The Challenge
Providers were spending significant time on manual documentation, either typing notes during the visit — which pulls their attention away from the patient — or writing them up afterward, which adds to the day's workload and contributes to after-hours charting. Any AI-driven approach needed accurate transcription that could reliably tell provider and patient speech apart, plus a way to turn a raw, informal conversation into something that actually reads like a clinical note rather than a wall of transcript text.
The Solution
We built an ambient documentation pipeline that captures the audio of the provider-patient conversation, transcribes it with speaker diarization through AssemblyAI so provider and patient dialogue are correctly separated, and then uses GPT-4 to draft a structured clinical note from that transcript. The note is delivered as a draft — the clinician reviews, edits, and signs off before anything becomes part of the official record, keeping a human firmly in the loop at every step.
